Preferred Specimen
2 Greiner Bio-One Vacuette – partial fill blood collection tube (3.2% Sodium Citrate tube) and one plain red top.

Collect 3 Greiner Blue tops if BOTH P2Y12 and Aspirin Resistance are ordered

2 mL in Red top
2 mL in each of Special Greiner Blue top tubes

Instructions
Venipuncture Collection
  1. Identify a site that is free of any peripheral venous infusions.
  2. Apply the tourniquet and identify the phlebotomy site.
  3. Release the tourniquet and cleanse the phlebotomy area.
  4. Reapply the tourniquet just before collection. It is important to minimize the amount of time that the tourniquet is applied prior to filling the tube in order to avoid activating the platelets while obtaining the sample.
  5. A 21 gauge needle or larger must be used to avoid hemolysis.
  6. Collect specimens in the following order: A Plain red top with at least 2 mL of whole blood must be drawn prior to drawing the Special Greiner Blue top tubes. Special Greiner Blue top tubes: If a single test is ordered: Collect 2 (Two) Special Greiner Blue top tubes. If both Aspirin Resistance Test and P2Y12 are ordered: Collect 3 (Three) Special Greiner Blue top tubes. (Note: the Special Greiner Blue top tube partially fills with 2.0 mL of blood, indicated by small black line. Do Not Overfill.) Note: If a CBC is to be collected at the same time, the CBC should be drawn last.
  7. Immediately gently invert the blue top tube 5 times. 
  8. Label the red and blue top tubes with Cerner labels. Write the collection time and collector ID on each tube. 
  9. All samples must be hand delivered to the lab (DO NOT SEND SAMPLES THROUGH THE TUBE SYSTEM).
  10. All samples must be kept at room temperature.

Central Line Collection:
  1. Ensure that the Central line is free of clots.
  2. Obtain a discard of at least 5 mL in order to clear the line.
  3. Obtain the sample for testing with a second syringe.
  4. Immediately transfer in the following order: 2.0 mL of blood into a plain red top tube Special Greiner Blue top tube: If a single test is ordered: Collect 2 (Two) Special Greiner Blue top tubes. If both Aspirin Resistance Test and P2Y12 are ordered: Collect 3 (Three) Special Greiner Blue top tubes (Note: the Special Greiner Blue top tube partially fills with 2.0 mL of blood, indicated by small black line. Do Not Overfill.)
  5. Immediately gently invert the blue top tube 5 times.
  6. Label the red and blue top tubes with Cerner labels. Write the collection time and collector ID on each tube.
  7. All samples must be hand delivered to the lab (DO NOT SEND SAMPLES THROUGH THE TUBE SYSTEM).

Clinical Significance
The Verify Now Aspirin assay is a qualitative assay to aid in the detection of platelet dysfunction due to aspirin ingestion in citrated whole blood for the point of care or laboratory setting. This assay is not for use in patients with underlying congenital platelet abnormalities, patients with non-aspirin induced acquired platelet abnormalities or in patients receiving non-aspirin anti-platelet agents (may be used in patients treated with selective COX-2 inhibitors, e.g. Celebrex).
