Instructions
- The transfusion should be stopped and a specimen should be drawn as soon as possible if a transfusion reaction is suspected.
- Call Blood Bank as soon as possible at:
- BFCMC 821-4350
- NLMC 820-1911
- NCMC 810-6430
- The transfusion container with remaining contents, along with unit tag with patient vitals and back of tag completly filled out including: symptoms, clerical error check, transfusion record medical record number, hospital ID band medical record, and blood bank armband medical record number should be returned to the laboratory.
Per Transfusion Reaction Workup protocol, order culture-C TRANS for the patient who received the transfusion
- DIN is entered under ‘Additional Source Information”
- Do not perform Gram Stain-NLMC Micro will perform gram stain.
- TRW must be ordered.
