10 mL whole blood in original Lavender top (EDTA)

5 mL (0.5 mL pediatric)

Instructions
  • The transfusion should be stopped and a specimen should be drawn as soon as possible if a transfusion reaction is suspected.
  • Call Blood Bank as soon as possible at:
    • BFCMC 821-4350
    • McKee 820-1911
    • NCMC 810-6430
    • SV 810-2570
      • SV should notify NCMC that a transfusion reaction workup will be coming and provide any patient information or details. 
  • The transfusion container with remaining contents, along with unit tag with patient vitals and back of tag completly filled out including: symptoms, clerical error check, transfusion record medical record number, hospital ID band medical record, and blood bank armband medical record number should be returned to the laboratory.
