Instructions
Specimen must arrive within 1 hour of collection between 7 a.m. and 1 p.m., preferably Monday through Friday. Instruct patient to collect specimen as follows: 1. Have patient collect specimen after a sexual abstinence period of a minimum of 48 hours and not longer than 7 days. 2. Collection is to be performed by masturbation. 3. Patient should ejaculate entire specimen into a screw-capped, sterile container without touching inside of container. 4. Close lid tightly. 5. Label container (not lid) with patient’s full name and date and time of collection. 6. Maintain sterility and forward promptly. Keep at body temperature. Note: Specimen collected as follows is not acceptable. 1. Orally 2. In a condom 3. Coitus interruptus 4. Utilizing a lubricant
