Preferred Specimen
1.5 mL serum from a serum separator tube (SST). Centrifuge as soon as possible after clot formation. Due to potential assay inferference, do not collect samples from patients receiving high dose biotin therapy (i.e., >5mg/day) until at least 8 hours (optimally 24 to 72 hours) following the last dose.

Clinical Significance
The reverse algorithm (Syphilis Screen w/rflx RPR and Titer or TPPA) begins with a treponemal specific antibody test, reactive results are confirmed with an RPR screen. If the RPR is positive, then it is an active infection. If the RPR is negative, then a secondary treponemal test (TP-PA) is employed. A positive TP-PA indicates a previously treated infection or latent syphilis; a negative suggests that syphilis screen was falsely positive and a syphilis infection is unlikely.
