Preferred Specimen
Pure culture of isolate submitted in Lowenstein-Jensen, Middlebrook 7H10 agar slant/plate or equivalent, or positive AFB broth medium (requires subculture prior to testing), safely contained

Instructions
Identification of isolate is required.
Must be submitted in a double-walled container or in 95K bag under room temperature.
