CPT Codes
81275; 81276; 81235; 88381 x2; If reflexed to ALK and ROS1, add 88377 x2

Preferred Specimen
Paraffin block

Minimum Volume
Seven (7) unstained slides

Other Acceptable Specimens
Nine (9) unstained tissue slides, minimum of seven (7) slides dependent on tumor tissue area.

Transport Temperature
Room temperature

Specimen Stability
Room temperature
  • Paraffin blocks are stable up to 10 years
  • Cut slides are stable for up to 60 days

Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Absence of tumor cells or decalcified specimens will be rejected.

Methodology
  • RGFR & KRAS by Real-Time Polymerase Chain Reaction (RT-PCR).
  • If reflexed, ALK & ROS1 by Fluorescense In Situ Hybridization (FISH).

Setup Schedule
  • EGFR and KRAS performed Monday - Friday
  • ALK and ROS1 set up on Monday at 10:00 PM

Report Available
  • EGFR & KRAS results within 1-3 days of receipt at the performing laboratory.
  • Reflex ALK & ROS1 results within 2-7 days of receipt at the performing laboratory.

Reference Range
No mutations detected.

Clinical Significance
Reflexive panel begins with EGFR and KRAS by PCR for mutation detection. If both EGFR and KRAS demonstrate a wild type status, then reflex testing to ALK and ROS1 by FISH will be performed.
