Clinical Significance

IgVH mutation status is an important predictor of survival outcomes in patients diagnosed with B-Cell chronic lymphocytic leukemia (CLL). “Unmutated” IgVH (greater than or equal to 98% homology with germline gene sequence) is associated with poor prognosis and significantly decreased survival compared with “mutated” IgVH, irrespective of the stage of the disease. Additionally, VH3-21 gene usage is associated with poor outcomes regardless of the IgVH mutation status. IgVH mutation status is also a predictor of high response rates and improved overall survival in patients treated with FCR (fludarabine, cyclophosphamide, and rituximab).
