Clinical Significance
Cascading starts with the most likely clinical disease associations and then moves to the least likely, in three tiers of assessment.:
Tier 1 – dsDNA, chromatin, Sm, Sm/RNP, RNP (Systemic Lupus Erythematosus, SLE/and Mixed Connective Tissue Disease/MCTD)
Tier 2 – SS-A, SS-B, Scl-70, Jo-1 (Sjogren Syndrome, Systemic Sclerosis, and Polymyositis)
Tier – 3 Centromere B, Ribosomal P (CREST syndrome and Neuropsychiatric SLE)
Serological antibody cascade used Bio-Rad BioPlex 2200 multiplex technology. Serology testing is not in itself diagnostic; connective tissue disease diagnosis requires clinical evaluation as well. If the ANA IFA test is positive but the included antibody tests are not detected, correlate with clinical findings to guide further investigation.
If overlap syndrome is being considered, separately-orderable versions of all of the serology tests are available for clinical use.
