Preferred Specimen

Instructions
Required for testing 2 mL blood. Avoid freeze-thaw cycling. Sample must be centrifuged within 2 hours of collection.
Serum must be removed from the red cells and put into aliquot tubes following centrifugation

Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Turbid, lipemic, grossly hemolyzed or heat-inactivated specimens. Cadaveric specimens or body fluids other than serum.

Clinical Significance
Test potentially useful as adjunct for diagnosing and monitoring of pancreatic cancer. May be used for differentiating patients with
cholangiocarcinoma and primary sclerosing cholangitis (PSC) from those with PSC alone. This test is not intended for screening of healthy/asymptomatic populations. Smoking increases serum CA 19-9 levels. CA 19-9 may be elevated in
patients with nonmalignant diseases including rectal polyps, diabetes, cirrhosis, hepatitis, other GI diseases and renal disease.
Lewis negative patients (5-7% of patients) do not express CA 19-9 due to the lack of the enzyme fucosyltransferase needed for CA 19-9
production. In these individuals, a low or undetectable serum CA 19-9 concentration is not informative regarding cancer recurrence despite
having a disease associated with elevated CA 19-9 levels.
