Patient’s red cells are tested using polyspecific antihuman globulin antisera (anti-IgG and C3). If positive, then test will be performed using monospecific reagents specific for IgG and C3. An elution and antibody identification will be performed on positive DAT specimens.

Clinical Significance
This test is used to identify in-vivo coating of RBCs with IgG and/or the complement component C3d which can occur in conditions
including autoimmune hemolytic anemia, hemolytic transfusion reactions, and drug-induced hemolytic anemia.
Interpretation:
Negative: No IgG antibody or complement (C3d) detected on the surface of the red cell.
Positive: IgG or complement (C3d) is present on the surface of the red cell.
Cautions:
- Autoimmune hemolytic anemia can occur with a negative direct Coombs test.
- The direct Coombs test can be positive in some apparently healthy individuals.
