  1. Provider contacts WV Division of Epidemiology (304-558-5358) prior to patient draw
  2. Provider completes "Arbovirus Test Request Form" with patient information, clinical findings and notes "Zika" in "Suspected Virus:" section
  3. Collect serum sample (single red or gold top tube), see "Arbovirus Specimen Collection Instructions"
  4. Send sample to WVUH Laboratory who will process and ship sample as described below
