HEPARIN DEPENDENT ANTIBODY WITH SEROTONIN RELEASE REFLEX

Message
This is the primary test used to diagnose heparin-induced thrombocytopenia (HIT) in conjunction with clinical parameters. A
serotonin release assay will be performed when resutls of this test do not correlate with clinical impression.

Preferred Specimen
One 3 mL BLUE top tube


Minimum Volume
1 mL Plasma


Instructions
Do not collect through heparinized line. Must be collected by venipuncture. Blue top tube must be filled properly to etched line to perform testing.
Samples must be centrifuged at 3000 rpm for 10 minutes or using a high-speed centrifuge.  Platelet poor plasma should be separated into 1 ml aliquots.  Samples should be frozen within one hour (best practice), however samples stable for 4 hours.  Samples must remain frozen during transport. Any samples that are received thawed are not suitable for testing.

Frozen (two 1 mL plasma aliquots) 


Specimen Stability
Ambient: 4 hours 
Frozen: 6 months (-70°C), freeze within 1 hour of collection. 


Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Tube contaminated with heparin, gross hemolysis, specimen clotted or not filled properly.

Latex-enhanced immunoturbidimetric method performed on ACL TOP analyzers.

Clinical Significance
Pathology will discuss positive results with provider. A recommendation for SRA testing will be detereminde at that time. SRA
testing, if indicated will be ordered by Pathology. Sample collection will be performed by provider.
