SUBXONE CONFIRMATORY/ DEFINITIVE, URINE, BY LC-MS/MS

Message
Definitive compliance checks in patients prescribed buprenorphine, naloxone, and/or suboxone
Confirming presumptive positive screening results for buprenorphine
[Urine quantitative values do NOT correlate well with serum concentrations and should not be used for assessing dose efficacy or timing.]

Test Code
LAB1230624

CPT Codes
80307

Includes
Adulteration testing for urine creatinine is provided at no-charge and included in the report.

Preferred Specimen
Urine, random specimen clean plastic collection cup.

Minimum Volume
5mL

Instructions
Specimens collected in clean plastic collection cups are acceptable. Store refrigerated for best stability.
Transfer mixed urine specimen to a labeled red top tube and deliver to Chemistry department for track-based processing, adulteration testing,
and aliquoting for use in the mass spectrometry section.

Transport Temperature
Refrigerated

Specimen Stability
Ambient: 24 hours   Refrigerated: 10 days

Methodology
LS-MS/MS

Setup Schedule
Monday - Friday dayshift

Reference Range
Negative

Clinical Significance
Quantitative/definitive detection of both buprenorphine (and its primary metabolite, norbuprenorphine) and naloxone, with flexible interpretations to give results for true exposures to suboxone, buprenorphine, and naloxone, and simulated compliance (aka, film dipping or pill-shaving) for buprenorphine and suboxone. Quantitative results are only provided for positive results; negative results will have an interpretation of “Negative” and a component result of “Not detected”.
All results require interpretation with the patient’s clinical context (ie, disease states) in mind. Creatinine: results <30 mg/dL generally suggest that the specimen is overly dilute. This can result in falsely low drug screen results.

Cutoffs:
Buprenorphine, 10 ng/mL
Naloxone, 5 ng/mL
Norbuprenorphine, 10 ng/mL
