BOTULISM LABORATORY CONFIRMATION
 
Steps to be taken by the patient care team:
Failure to follow instructions will delay samples from being sent to the CDC for testing.

1. Contact the WV State Epidemiologist at the WVU State Hygienic Department in Charleston (304-558-3530) after obtaining a complete patient history on a patient history form from the requesting physician. 

 
NOTE:  The Infant Botulism Treatment and Prevention Program – CDPH (California Dept of Health) is where the BabyBIG® product is obtained.  This approval from California does not substitute for contacting the OLS in West Virginia for CDC testing approval.
 

2. The State Epidemiologist must approve testing prior to sending the samples to the Center for Disease Control in Atlanta, Georgia. Samples are sent to the WV State Health Laboratory and forwarded to the CDC. 

 

3. Refer to the CDC website for the form and testing instructions.

Once specimen(s) are received in lab, microbiology is also required to contact the WV state office of lab services and provide them with the following information
Collection method:  stool vs stool collected by saline enema
Collection of stools:  before or after administration of BabyBIG®.
Date of BabyBIG® Administration:
Approval information from the WV State Epidemiologist for sample submission approval.

HELP Lab order: Specify: CDC-10132
Once specimen(s) are received in lab, if the microbiology is granted approval, then samples will be submitted based on the instructions of the OLS laboratory.

Serum 5mL (red top or gold top) before anti-toxin administration
Fresh stool in leak-proof container (10 g)
Enema (with sterile non-bacteriostatic water)
Food in original container (unopened commercial products NOT tested)

Foodborne botulism: serum, stool, enema, food. Although not ideal, gastric contents may also be submitted.
Wound botulism: serum, debrided tissue, swab from wounds, stool (only if foodborne is also suspected).
Infant botulism: stool, enema, rectal swabs, potential sources (honey, opened formula, etc.)

Delivered to laboratory ASAP, refrigerate upon receipt in lab

Specimens shipped to CDC on Mon-Thursday ONLY. 

Final written reports are provided within 12 weeks. Preliminary results may be available within 48 hours of specimen receipt.
