Clinical Significance
Reticulocyte hemoglobin content (RET-He) measures reticulocyte hemoglobin content and can be used to assess hemoglobin available for mature RBC production. Immature reticulocyte fraction (IRF) measures the most immature forms of reticulocytes exhibiting high nucleic acid content. Depending on the appropriate clinical context, one or both of these parameters may be used to characterize erythropoietic activity in anemic patients. Some specific scenarios include the evaluation of absolute and functional iron deficiency, response to erythropoietin stimulating agents, and the evaluation of peripheral loss/destruction or marrow suppression of red blood cell production. Anemic patients with elevated IRF and/or RET-He values suggesting that RBCs are being actively produced may not require a RBC transfusion depending on the clinical situation.
