Instructions
Patient Preparation:

1. Pharmokinetic factors including dosage form, mode of administration, coadministered drugs, age, renal function, and individual variations in adsorption, distribution, and excretion can influence appropriate time of specimen draw following administration of vancomycin.



2. Trough monitoring should begin after steady-state is achieved and specimen should be drawn within 30 minutes of next dose.



3. When peak level is being measured, specimen should be drawn 0.5 to 2 hours after infusion.
