Clinical Significance
Lidocaine is an anti-arrhythmic drug commonly used to restore a regular heartbeat.
The therapeutic effectiveness and toxic side effects of lidocaine in the treatment of cardiac arrhythmias correlate better with the serum concentration than with the administered dose.
After suspected myocardial infarction, monitoring lidocaine in serum during infusion helps to establish and maintain adequate prophylactic or therapeutic drug concentrations and avoid toxicity. However, lidocaine concentration determinations are most useful when the result is obtained soon after the sample is drawn because the drug's half-life is very short (74 to 140 minutes), and the patient's potentially changing condition can unpredictably alter the drug's distribution, metabolism and elimination.
