Included in Lupus Anticoagulant Profile. Oral anticoagulants will prolong test. Abnormal DRVV Screen reflexes DRVV Confirm. Outreach clients should double-spin the blue top plasma, remove the plasma from the RBC button, and freeze in a polypropylene tube.

Both the DRVV screen and the DRVV confirm should be performed at the same time and on the same sample.

Clinical Significance

Lupus anticoagulants are antibodies directed against phospholipid/protein complexes.  They have the ability to prolong the clotting times of the phospholipid-dependent tests. Lupus anticoagulants are associated with numerous clinical states such as: auto-immune diseases (systemic lupus erythematosus), thromboses, recurrent spontaneous abortions and infections.  Their presence may be persistent or transitory. The Russell’s viper venom is present in the dRVV Screen and dRVV Confirm reagents.  This venom acts in the presence of calcium as an activator of factor X and thereby triggers the coagulation cascade downstream from factor X, thus eliminating the influence of coagulation factors acting upstream.  This test is not affected by the contact factor anomalies or by factor VIII and IX deficiencies or inhibitors. The dRVV Screen test is performed with a low concentration of phospholipids.  If LA are present, the clotting time will be lengthened. The dRVV Confirm test is performed with a higher concentration of phospholipids which neutralize the LA present in the plasma being tested.  Therefore the clotting time obtained with the dRVV Confirm will be shorter than the one observed with the dRVV Screen.
