Preferred Specimen
1 slant/plate/broth pure culture of isolate safely contained in Lowenstein-Jensen or Middlebrook 7H10 agar slant/plate or equivalent or positive AFB broth medium isolate safely contained in a double-walled container


Instructions
Isolate must be submitted in 95k bag.
Identification of the isolate is required.
