Dependent upon testing performed and level of complexity of pathologist review.

Includes
Microscopic interpretation by a pathologist; includes H & E slide preparation if paraffin blocks are submitted.  If paraffin blocks are submitted, then Special stains (CPT code(s): 88312 [each stain] or 88313 [each stain]) and/or IHC immunohistochemistry stains (CPT code(s): 88342 for each antibody) may be ordered at the discretion of the pathologist. These services will be performed at an additional charge.


Preferred Specimen
Formalin-fixed, paraffin-embedded tissue block or 4 unstained slides, submitted in a slide folder


Other Acceptable Specimens
H & E slides submitted in a slide folder

Required information: Patient history and any other relevant information; CBC results; peripheral blood smear; name, telephone # and fax # of referring physician.
