If culture is positive, identification will be performed at an additional charge for each organism (CPT code(s): 87106 or 87107.
In the event of an initial identification of Coccidioides, Histoplasma, or Blastomyces, the following additional identification methods will be performed at an additional charge: Coccidioides DNA Probe (CPT code: 87149); Histoplasma DNA Probe (CPT code: 87149); Blastomyces DNA Probe (CPT code: 87149).
If an additional identification method is required, ITS nucleic acid sequencing may be added at an additional charge (CPT code: 87153).
Yeast or Mold Susceptibilities will be added upon request.

Ear, oral, throat, nose, nasopharyngeal, wound or genital specimen collected by swab and transported in a liquid or gel transport medium, or
Ocular specimens collected in a sterile, leak-proof container, or transport swabs in liquid or gel transport medium, or
Sputum, bronchoalveolar lavage (BAL) fluid, brushing, washing or aspirate collected in any sterile leak-proof container, or
Tissue or biopsy specimen submitted in a sterile, leak proof container, or
First morning, clean catch midstream urine Catheter collected, surgically collected urine, or after prostatic massage
Ear, oral, throat, nose, nasopharyngeal, wound or genital specimen collected by swab and transported in a liquid or gel transport medium, or
Ocular specimens collected in a sterile, leak-proof container, or transport swabs in liquid or gel transport medium, or

Please note: This Test Code should be used for specimens from sources other than blood or specimens found on the skin surface.
Fungus cultures from all skin, hair, and nail sources should be ordered using Test Code 39515X (Fungus Culture {Skin, Hair and Nails}), and cultures on blood should be ordered as 4606X (Fungus Culture, Blood), or 4605X Culture, Fungus, Skin, Hair Nail w/ Direct Fluor/KOH.
Clinicians who send tissues, biopsies, or aspirates for deep fungus should label the specimens as such, and continue to order Test Code 105 (Fungus Culture, Sources other than Skin). Please contact the Microbiology Department for any other questions.

Clinical Significance
Fungi can infect the cerebral spinal fluid, other body fluids, and tissues. Infections occur more commonly among patients who are immunocompromised. Early diagnosis and effective treatment can reduce the likelihood of severe sequalae.
