Cryoglobulin, Qualitative Analysis

Test Code
383

CPT Codes

Preferred Specimen
3 mL fasting serum collected in a red-top tube (no gel)

Patient Preparation
Overnight fasting is required

Minimum Volume
2 mL

Instructions
Collect 6 mL of fasting whole blood specimen in a red-top tube (no gel). 3 mL serum required: 2 mL minimum. Avoid hemolysis.

Allow serum sample to clot for one (1) hour in a 37° C water bath, oven, incubator, or heat block. After clotting, centrifuge the specimen for 10 minutes at 1600 RCF(G) utilizing one of the 3 options below:
1) In a "warm" centrifuge at 37° C.
2) In a bench-top centrifuge placed in a "warm room" at 37° C.
3) At room temperature, in a bench-top centrifuge prewarmed by running it for 10 minutes at 1600 RCF(G).
After centrifuging, decant serum into a screw-cap vial.
Transport at room temperature.

Note: If the sample is NOT kept at 37° C during all processing steps, the cryoprecipitate if present can precipitate out of solution and be lost in the clot or during the centrifugation process and result in a false negative result.
Transport screw-capped vial at room temperature.
Do not refrigerate or freeze.

Note: No tests can be added or poured off this tube; cryocrit and cryoglobulin evaluation only

Transport Temperature
Room temperature

Specimen Stability
Room temperature: 72 hours
Refrigerated: Unstable
Frozen: Unstable

Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Gross hemolysis • Grossly lipemic • Received refrigerated • Received frozen • Icteric samples • Serum separator tube (SST)

Methodology
Visual Inspection

Setup Schedule
Monday-Sunday Evening
Report available: 4 Days

Reference Range
None detected

Clinical Significance
Cryoglobulins are a group of proteins which precipitate from serum, plasma or urine upon exposure to cold.
