Clinical Significance
Autoantibodies to brain neurotransmitter cell surface receptors may be seen in paraneoplastic syndromes or in idiopathic autoimmune encephalitis. Antibodies to N-methyl-D-aspartate receptor (NMDAR1) may be seen in a progressive syndrome leading to behavioral changes, psychosis, seizures and coma; teratoma or other neoplasms are seen in some cases. Antibodies to voltage gated potassium channel (VGKC) are found in paraneoplastic and idiopathic autoimmune encephalopathy; in many cases, antibodies actually target VGKC-associated proteins LGI-1 or CASPR2. Antibodies to AMPAR1 and AMPAR2 are usually associated with limbic encephalitis but can present with other symptoms or psychosis, and is paraneoplastic in >50% of cases.
