Complete the following information: A. Request filled by B. Today’s Date C. Operative Date D. Nephrologist E. Radiologist F. Specimen Submitted (identify laterality and native vs. transplant) G. Surgical procedure and findings H. Clinical history/diagnosis I. Labeling check-off list J. Time Specimen removed from patient K. Utilize pathology information in the comments field. L. Add additional information for cases with more than 11 specimens. M. May utilize Pathology Communication Order for additional information after first order is placed.

Specimen Stability

BUMCP Deliver specimens and requisition and any other pertinent clinical information ASAP to the Surgical Pathology Lab at Banner University Medical Center Phoenix. If it is an after-hours delivery, take to the main Lab - Test Management processing area
