Transfuse PRBCs (Type and Cross); Add Comment: "GRANULOCYTES"

Performed in Blood Bank.  A "Physician Approval to Transfuse Under Abnormal or Unusual Circumstances" form must be signed by the physician for Granulocytes to acknowledge incomplete donor testing on the product.

Preferred Specimen
A current type and screen must be performed prior to ordering the product from the blood supplier in order to find an appropriate donor.  Call Blood Bank for clarification.  Granulocytes must be crossmatched after they have been received from the blood supplier.


Instructions
Granulocytes are ordered through the Transfuse PRBCs Advisor with an order comment "Granulocytes."  The following special ordering instructions MUST be included on the requisition: 
  • HLA Testing: Attention to HLA compatibility is also required for allo-immunized recipients. 
  • Irradiated: Granulocytes must always be Irradiated to prevent GVHD. 
Ordering Comments, if applicable: 
  • CMV Seronegative: Must be approved by Laboratory Medical Director.
PRODUCT EXPIRATION/TRANSFUSION:
Granulocytes expire 24 hours after collection from the donor.


Setup Schedule
Granulocytes must be ordered from the blood supplier and are not available as a stock blood component.  They require a minimum of 24 hours notice prior to transfusion to allow preparation of the donor.  Call the Blood Bank for specific information on availability.

TRANSFUSION: Granulocytes are recommended for the treatment of neutropenic patients who have documented infections, especially gram-negative bacteria and fungus, and who have not responded to antibiotics.
