Clinical Significance
Sodium nitroprusside is a potent hypotensive agent. It is used in the treatment of malignant hypertension and as an adjunct in the control of blood pressure in surgery. The major metabolite is thiocyanate which is largely excreted by the kidneys. Thiocyanate levels should be monitored in any patient receiving therapy longer than 48 hours, or who has renal or hepatic impairment. Thiocyanate is also a metabolite of acetonitrite, acrylonitrite, amygdalin and cyanide. Thiocyanate is higher in smokers vs. non-smokers.
