Susceptibility to Clindamycin will be performed and reported only when the patient is Penicillin allergic. (CPT code 87184)


Preferred Specimen

  Swab the lower vagina (vaginal introitus), followed by the rectum (i.e., insert swab through the anal sphincter) using the same swab.
Cervical, perianal, perirectal or perineal specimens are not acceptable.
A speculum or lubricant should not be used for culture collection.
Label tube with patient identification , specimen source and Collection date
 




Instructions
  1. Take one or two swabs of the lower vagina (vaginal introitus) and the anorectum. Do not use a speculum or lubricant . Send both swabs together.


Transport Container
Place the swab(s) into a nonnutritive transport medium. When feasible, specimens should be refrigerated before processing.  


Transport Temperature

Room Temperature: <24 hours




Specimen Stability

GBS isolates can remain viable in transport media for several days at room temperature; however the recovery of isolates declines over one to four days, especially at elevated temperatures, which can lead to false-negative results.


 


Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Cervical, perianal, perirectal or perineal specimens are not acceptable, and a speculum should not be used for culture collection. 


Methodology
Culture enrichment for Group B strep (CPT 87081) and Loop-mediated isothermal DNA Amplification (CPT 87653) (LAMP)  

Setup Schedule
Test performed Sunday through Saturday 6am- 11pm, twice per day.  Results reported daily as performed .


Limitations
Negative test results may occur from improper specimen collection, handing or the number of organisms is below the analytical sensitivity of the test.


Reference Range
Negative for Group B Streptococci


Clinical Significance
  • Prenatal screening of pregnant women for carriage of Group B Streptococci.
  • Maternal intrapartum GBS colonization is the primeary factory for early-onset GBS disease in infants
  • in 1996 the American College of obstetricians and Gynecologists(ACOG) recommended a risk based approach fro the intra-partum prophylaxis for all women in labor, who had identifiable factors associated with increase risk for delivering infants who will develop GBS disease.
  • The selective brth enehancement method followed by the buckeic acid ampification test (NAAT) is the most sensitive method for the rapid detection ogf GBS. if the patient is allergic to peniclin and an alternate regimen of chemoprophylaxis is required. susceptibility testing to Clindamycin will be performed.
