Instructions


Once the Tissue specimen has been removed , place the Tissue Specimen into a Container of 10% Formalin.

Label the Specimen Container with the Patient's Full Name , Date of Birth and/or Medical Record # , Date of Collection
and the Site(s)  of the Specimen(s).

Please complete a Surgical Order Form ( # 230 ) with the Pertinent / Required Information and submit it with the Patient Specimen.
