Instructions
Collect the Cytology Sample using either a Broom-like Device or a Combination Brush / Plastic Spatula with Detachable Heads.
Follow the Manufacturer's Collection Instructions for the Detachable Head Device(s).
Drop the Detachable Head Device(s) into the Surepath Vial.
Place the Cap on the Vial and tighten securely.
Label the Vial with the Patient's Full Name , Date of Birth and Site of the Specimen.
Please complete a Cytology Order Form ( # 240 ) with the Pertinent / Required Information and Submit it with the
Patient Specimen.
Follow the Manufacturer's Collection Instructions for the Detachable Head Device(s).
Drop the Detachable Head Device(s) into the Surepath Vial.
Place the Cap on the Vial and tighten securely.
Label the Vial with the Patient's Full Name , Date of Birth and Site of the Specimen.
