2 Gray top tubes (potassium oxalate/sodium fluoride)


Instructions
Draw blood in a grey-top (potassium oxalate/sodium fluoride) tube(s) and send 0.5 mL of potassium oxalate/sodium fluoride whole blood refrigerated. Note: 1. DO NOT use alcohol prep to cleanse skin. Use a NON-ALCOHOL GERMICIDE to cleanse the skin prior to venipuncture. 2. Please draw specimen using a Legal Alcohol Collection Kit and complete a "Chain-of-Custody Request Form."
