NOTIFY ANATOMICAL PATHOLOGY IN ADVANCE- 725- 4817

For non-Epic users or Epic users during a downtime

For SRH staff:  Please submit an appropriate downtime form or a paper requisition. Include:  patient's name, social security number, address, birthdate, patient's sex, specimen source (right or left side, if applicable), operative diagnosis, requesting physician, date and time specimen was collected. 


For Outside offices:  Submit a Requisition for Tissue Examination form.  For insurance information, submit a copy of insurance card or a form from another information system.  Requisition must Include:  patient's name, social security number, address, birthdate, patient's sex, specimen source (right or left side, if applicable), operative diagnosis, requesting physician, date and time specimen was collected.  
 

Container must be labeled with patient's name, birth date, date of collection, requesting surgeon, specimen source

Transport Temperature
Maintain specimen at room temperature.


Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Specimen submitted in fixative, water or saline; improper labeling


Setup Schedule
Monday - Friday 6:00 AM until 4:30 PM

After hours of Mon-Fri 7:00 - 4:30 and on weekends:  Immediately notify the pathologist and anatomical lead tech.
