SRHC Clients:

Blood should be collected at least 24 hours after birth.  If a patient is discharged early, specimens from infants receiving only non-lactose containing feedings must have the form clearly marked as such.
Cord Blood is not acceptable.

SPECIAL CIRCUMSTANCES:
 Transfusion:
If the infant is to undergo a transfusion of blood products, a specimen should be obtained prior to the transfusion.  If this is not possible, collect the specimen and mark the form “Transfused Yes” indicating the date of the most recent or last transfusion.  Infants who receive transfusions should have a repeat hemoglobinopathy screening 2 months after the date of the last transfusion.
Do not mark the “Transfused Yes” box if the transfusion took place 2 months before the collection date.
 
Premature Infants:
In premature infants, newborn screening results can be falsely abnormal.
If the infant is on a lactose containing feeding, the specimen should be collected at least 24 hours after birth.
If the infant is receiving only IV fluids or total parenteral nutrition, the specimen may be collected at least 24 hours after birth and the DHEC form marked “TPN” or “NPO”.
All premature infants should receive their initial screening by 7 days of age regardless of their health status

Instructions
 1.  Complete the information requested on the DHEC Newborn Screening form.  If the SRH account number is entered as the Hospital DHEC number, the leading zero and hyphen should be dropped (e.g., 0319-000011 is entered as 319000011). 

 2.  Hold the infant’s limb in position to increase venous pressure.  A heel warmer may be applied for no more than 3 minutes to aid in getting an adequate sample. 


 3.   Cleanse the heel and wipe with antiseptic solution.  Allow the heel to air dry


 4.   Puncture heel with sterile lancet.  Gently wipe off first drop of blood with clean gauze, as this drop contains tissue fluid. 

5.     Wait for spontaneous flow of blood.  Apply gentle pressure with thumb and ease intermittently as a blood drop forms. 


6.     Touch the printed side of the filter paper to the blood drop only and not to the skin.  Fill each circle by making a single application of blood to the filter paper.  The blood should soak through the filter paper to the other side.

7.      Collect blood in all the circles.


 8.    Allow blood samples to air dry on a level surface for at least 4 hours or overnight.  Do Not stack or heat to dry. 


9.     Order the Neonatal screening in the computer.


10.  Send the completed DHEC form to Laboratory with “Nurse to Collect” form.  The “Nurse to Collect” form must include the patient’s name, a second unique identifier, date and time of collection, and nurse’s signature.
