Instructions

Adult: 4 mL in EDTA tube (Lavender or PInk) *Pediatric minimum: 2 mL in EDTA tube (Lavender or PInk)*

Transport Temperature

1. Order post transfusion urinalysis and transfusion reaction workup in computer. 2. Send properly labeled sample with existing typenex number or call laboratory for collection. 3. Hand Deliver completed transfusion reaction report form and entire unit with set up and attached IV fluid to blood bank NOTE: No further blood will be issued until workup is resolved.
