Instructions
The patient should be instructed to remove dentures, rinse mouth, and gargle with water. The patient should then be instructed to cough deeply and expectorate sputum into sterile screw-cap container. After the specimen has been collected, the specimen should be examined to make sure it contains a sufficient quantity (at least 1 mL) of thick mucus (not saliva).


Transport Temperature
Test Includes Isolation and identification (additional CPT code) of aerobic bacteria considered pathogenic in the lower respiratory tract. Susceptibilities are performed, at additional charge, when appropriate. Anaerobe culture is not appropriate from expectorated sputum. See requirements for Anaerobic Culture [008904]. CPT coding for microbiology and virology procedures often cannot be determined before the culture is performed. Special Instructions It is strongly recommended that a Gram-stained smear be performed on all expectorated sputa to determine the acceptability of a specimen for bacterial culture (see Gram Stain Evaluation With Sputum Culture Reflex [182352]). Sputum specimens should not be cultured under the test number for lower respiratory culture as this test does not include a Gram stain evaluation (unless a Gram stain has already been performed by the client at the time of specimen collection). Lower respiratory specimens cannot be cultured under the aerobic bacterial culture test number. If specimens are incorrectly submitted with an order for aerobic bacterial culture, the laboratory will process the specimen for the test based on the source listed on the request form. The client will not be telephoned to approve this change. After the specimen has been collected, the specimen should be examined to make sure it contains a sufficient quantity (at least 1 mL) of thick mucus (not saliva). Only the screw-cap container should be submitted to the laboratory. Storage Instructions Refrigerate at 2°C to 8°C
