Instructions
Patient must be fasting at least 4 hours. Special prechilled collection tube containing 2.5 mL of 6% perchloric acid (T012) required. Collect enough blood into syringe and add exactly 1 mL of blood to the prechilled collection tube and shake vigorously to mix. Please submit completed form with sample.
