Vancomycin specimens should be drawn just prior to a Dose. Note: Provide the date and time of last Dose of Vancomycin given. Collect a full tube, following standard venipuncture techniques.

Minimum Testing Volume: 0.5 mL Serum Transport Temperature: Refrigerated Stability: Ambient: 3 daysRefrigerated : 8 days (Specimen must be removed from the clot or RBCs)Frozen: 3 Months Causes for Rejection: Unlabeled, mislabeled, wrong tube type, QNS, severely hemolyzed samples, and any sample from which the lipid layer hasnot been removed.
