Check to make sure the PKU form has not expired. Check to make sure the metabolic screening (PKU) form has been completed, if not have the parent complete personal information and you complete physician information. Verify the name and date of birth on the PKU form against the physician order or the requisition. Warm the infant's heel for about three minutes to increase blood flow. Disinfect the puncture site with a 70% alcohol prep pad and allow to air dry. Puncture the skin with the Quick Heel lancet and wipe away the first drop of bloodAllow the second drop to form by the spontaneous free flow of blood, while using proper technique of squeezing and relaxing for additional drops to develop. Without handling the filter paper portion, touch the back of the circle, as close to the center as possible, to the drop of blood. While observing from the opposite side, allow the blood spot to enlarge until the circle is exactly filled. Fill the circle with a SINGLE application of blood on the filter paper from the heel. Do not apply blood to both sides. Fill all five circles. Complete the date and time of collection on the form. Sign using your collector ID.

Instructions
Allow the blood spots to air-dry thoroughly for at least four hours at room temperature. Keep away from heat and direct sunlight. Do not superimpose one wet filter paper on another or contaminate with papers or other objects. Do not refrigerate.
