Note:
1. Dosage time and route of administration are required.
2. Indicate serum and as peak or trough.
3. Label specimen appropriately (serum and as peak or trough).

Instructions
Draw trough specimen 30 minutes before next dose, and peak specimen 30 minutes after completion of infusion.
