Container/Tube: Slide(s)-Improperly labeled slide, poor quality slide, or broken slide is not acceptable.
Forms: Surgical Pathology Request Form
Collection Instructions: Collect 2 mL of urine in a sterile container. Submit cytology or histology slides. The attending physician may directly request the necessary material from the outside hospital or laboratory or may ask the Pathology Department to initiate the request.
Instructions
Note:
1. In cases where certain diagnostic studies, including x-ray examinations, histologic studies, electrocardiograms, chemical and bacteriologic data, etc. have been done elsewhere and which are being used as the basis for definitive treatment at CoxHealth Laboratories, either the original or a copy of the report is to be furnished by the attending physician.
2. In cases where the histologic diagnosis has been made elsewhere, written documentation signed by the staff physician is to be furnished to the Pathology Department at CoxHealth Laboratories in the form of a clinical note prior to submission of the specimen. Documentation should include the date of biopsy, laboratory, and pathologist involved in the diagnosis. Similarly, when diagnosis depends on X-ray or laboratory results done elsewhere, a copy of these reports must be affixed to the chart and signed by the staff physician.
3. It is strongly felt that all relevant outside slide material on patients undergoing treatment be reviewed by the Pathology Department at CoxHealth Laboratories.
