Preferred Specimen
Submit only 1 of the following specimens:

Plasma
Container/Tube: Green-top (heparin) tube(s)
Specimen Volume: 3 mL (minimum volume: 0.5 mL) of plasma
Collection Instructions: Trough level should be drawn 30 minutes before dose, and peak level should be drawn 2 hours after dose for fast release preparations and 4 hours after dose for slow release preparations. Random level is acceptable.
Note: 1. Dosage time and route of administration are required. 2. Indicate plasma and as peak, random, or trough. 3. Label specimen appropriately (plasma and as peak, random, or trough).

Serum
Container/Tube: Serum gel tube(s)
Specimen Volume: 3 mL (minimum volume: 0.5 mL) of serum
Collection Instructions: Trough level should be drawn 30 minutes before dose, and peak level should be drawn 2 hours after dose for fast release preparations and 4 hours after dose for slow release preparations. Random level is acceptable.
