Patient Preparation
Umbilical Cord Specimen Process:
CRMC RN to collect cord specimen on every delivery following the ARUP Collection Instructions:
  1. If the specimen is going for testing immediately following birth:
    1. Apply the newborn lab label for the toxicology test on the side of the specimen container with the date, time and initials of the collector.
    2. Place in biohazard bag and place in the pneumatic tube to send to the lab.
  2. If the specimen is going to “holding” for potential future testing then:
    1. Apply the newborn EPIC label to the side of the specimen container with the date, time, and initials of the collector.
    2. Write the date of the collection on the top of the lid
    3. Place in a biohazard bag in the storage bin in the soiled utility room for transport to the blood bank at midnight each night.
    4. Document in the specimen log
  3. At midnight the OB Tech will take the tray of all the cord specimens for that day to the blood bank and complete the following steps:
    1. The biohazard storage refrigerator in the lab will have 3 trays of cord specimens in three drawers labeled Day #1, Day #2, and Day #3.
    2. Remove the tray from the bottom Day #3 rack and dispose of all remaining specimens in the biohazard receptacle next to the refrigerator.
    3. Move the tray from the Day #2 rack to the Day #3 rack and the Day #1 tray to the Day #2 rack.
    4. Place the new tray of specimens from the current day onto the top rack labeled Day #1.
  4. If the need arises during the postpartum stay to complete toxicology testing on the umbilical specimen complete the following steps:
    1. Once ordered in EPIC, the staff member will take the newborn lab label to the blood bank biohazard refrigerator (same one that we store the cord blood in) and apply the lab label to the container with the date, time, and initials written on it.
    2. Take the specimen to the lab processing area for processing.


Instructions
Drain and discard any blood. Rinse the exterior of the cord segment with normal saline or water. Pat the cord dry and transport at least 8 inches of umbilical cord in a routine urine collection cup.
