Lower Respiratory Tract Specimens:  Sputum, ET Aspirate, Bronchial washings, Bronchial or tracheal suction material, Trans-tracheal aspirates, lung aspirates, lung biopsies.


Instructions
Follow this procedure if the physician has requested an outpatient specimen collection for AFB culture or sputum culture (or other sputum specimens):
ENGLISH:  Collection of Sputum Specimen:
Collect specimen when patient first gets up in the morning, before eating:
  1. Brush teeth with a wet toothbrush without toothpaste.
  2. Rinse mouth and gargle with water.
  3. Cough deeply from the lower chest and collect 5-10 ml sputum into the sterile container provided.
    • The testing your physician is requesting requires you to collect the specimen from a deep cough. Do not collect saliva (spit) from the mouth or postnasal discharge into the container.
  4. Screw the lid carefully onto the container, and place the container inside the “zip-lock-type” bag which the lab provided. The bag has the word “Biohazard” on the outside of the bag.
  5. Bring specimen to the lab within 1-2 hours. If there is a delay of more than 3 hours, refrigerate the specimen. Specimen must be delivered to the lab before 2:00 pm on the day it is collected.
  6. AFB specimens are not collected in the lab facility. These sputum specimens must be collected by the patient and delivered to the lab in a sterile container.
  7. Sputum specimens for pneumonia could be collected by the lab if necessary. Wear a protective mask in addition to usual protective equipment if you are assisting the patient.
 
INSTRUCCIONES PARA LA COLECTA DE FLEMA
Colecte la muestra de flema en la manana ontes de comer y ontes de lavurse la boca:
  1. Cepillese los dientes, sin pasta dental.
  2. Enjuagese la boca y hugu gorgusus con agua.
  3. Tosu profundumente, desde la purte buja del pecho y colecte de 5-10 ml de flema en el recipiente que se le dio.
  4. Cierre el recipiente, y pongulo udentro de la bolsu de plastico que se le dio en el laboratorio.
  5. Traigu la muestra al laboratorio en no mus de (2) dos horus despues de la colecta, si no puede traerla en 2 horas, montengo la muestra refrigerada, la muestra tiene que estor en el laboratorio untes de las (2) dos de la torde en el dia que fue colectada.
  6. La colecta de AFB no se hucen en el laboratorio, esta colecta de flema deve de ser colectada por el paciente y llevedu al laboratorio en un recipiente esterilizudo.
  7. La colectu de lu flemu para pneumonia puede ser colectadu por el laboratorio, si es que es necesario, use un a muscara protectora, a purte del equipo de proteccion, si usted esti assistiendo el puciente.

Specimen Stability
  • Transport to laboratory as soon as possible (<3 hours) at room temperature
  • If > 3 hours,
    • Sputum and ET aspirates:  refrigerate at 2-8 0 C.
    • Other specimens:  do not refrigerate, keep at room temperature.

Clinical Significance
Cultures of specimens from the lower respiratory tract may be useful in the diagnosis of pneumonia. The lower respiratory tract is sterile; however, the upper respiratory tract is colonized with bacteria. Collecting a specimen with the least amount of upper respiratory tract contamination will provide better results for diagnosing causative organism for pneumonia.
