Instructions
TIMING OF COLLECTION:
- If the physician gives specific instruction for timing of collection, the physician’s order should be followed.
- If no specific timing instructions, blood cultures can be drawn at the same time –however, the second set should be drawn from a different site (eg. left arm for first set and right arm for second set). Both sites must be prepared completely using the proper skin cleaning procedure. NOTE: volume of blood collected is more important than timing of the draws in most instances
- Drawing at a specific interval may be necessary for patients with suspected endocarditis – often the timing for this situation would be to draw 30 minutes apart.
- Timing of blood culture draws should be before patient is placed on antibiotics - eg. in the Emergency department before antibiotics are administered.
- If patient is on antibiotics, the blood cultures should be drawn when the antibiotic levels are at their lowest - just before the next dose.
- Adults and pediatric patients > 2 months of age: Vigoorously cleanse the venipuncture area with a Hygea Pesonal cleansing washcloth. Cleanse with alcohol wipes and allow dry time. Scrub 30 seconds with Chloraprep.
- Pediatric Patients <2 months: Use the alcohol wipes 2x
- NICU Patients:
- Weighing < 1500 grams, use 10% povidine iodine
- Weighing >1500 grams, use chloraprep

Critical Result: Positive Gram Stains on Blood Cultures.
Clinical Significance
Blood cultures are collected whenever clinically significant bacteremia is suspected. It is important to use the strict aseptic technique and supplies as outlined below when collecting the blood culture to prevent skin microorganisms from contaminating the culture.
