- Notify the physician (or RN in charge of patient).
- Hematology:
- Make additional slides to send to Public Health
- Complete the Public Health Laboratory Form and provide the Form and Slides to the send out department.
- Fill out the patient information
- Submitting Facility: CRMC
- Bill to: leave blank
- Send/Fax results: Enter 559-459-2319
- Check the box for P9 Parasitology Reference Specimen
- Origin of Specimen: Select "other" and write in Malaria ID.
- Send Out Department:
- Order a MISC Test: Malaria Slides to Public Health.
- Send to Public Health:
- The completed Public Health Laboratory Form,
- the slides,
- and a Patient history printout from EPIC that includes Presenting symptoms and travel history.
Clinical Significance
