Platelet Function Analysis

Test Code
PFA

CPT Codes
85576

Preferred Specimen
Citrate (BLU) Whole Blood

Instructions
Collect whole blood in 3.2% buffered sodium citrate (9:1 blood to sodium citrate). Venipuncture must be atraumatic.

Specimen Stability
Do not refrigerate or centrifuge samples. Samples must be stored at room temperature and are stable for up to 4 hours.

Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Hemolyzed specimens, short draw vacutainers, Clotted

Methodology
PFA-100

Setup Schedule
24x7

Reference Range
CEPI:  Collagen/Epinephrine:  80-184 Seconds
CADP:  Collagen/ADP:              56-102 Seconds

If the Collagen/Epinephrine (Col/EPI) test cartridge is abnormal, the sample is tested in the Collagen/ADP (Col/ADP) to rule if the abnormal result in the Col/EPI was caused by the effect of ASA or medications containing ASA.
The following are expected patterns observed with the PFA test:
  NORMAL ASA VWD GLANZMANN’S
THROMBASTHENIACADP
Col/EPI Normal Abnormal Abnormal Abnormal
Col/ADP Normal Normal Abnormal Abnormal

Clinical Significance
The PFA-100 system is an instrument and test cartridge system in which the process of platelet adhesion and aggregation following a vascular injury is simulated in vitro. Platelet dysfunction detected by the PFA-100 system may be acquired, inherited, or induced by platelet inhibiting agents. The most common causes of platelet dysfunction are related to uremia, von Willebrand disease, and exposure to agents, such as acetyl salicylic acid (ASA, for example aspirin).

Performing Laboratory
