Clinical Significance
The normal cerebrospinal fluid (CSF)-to-serum glucose ratio is about 0.6. CSF from the ventricles of the brain has a higher glucose level than CSF obtained from lumbar puncture (by about 6 to 18 mg/dL). Determination of CSF glucose helps distinguish bacterial from viral meningitis; the glucose value is often low (less than 40% to 45% of simultaneously analyzed, equilibrated serum glucose) in bacterial meningitis and tuberculous meningitis and is generally normal in viral meningitis (though viruses associated with meningoencephalitis, such as Herpes, can be associated with low CSF glucose). Carcinomatous meningitis (widespread infiltration of the meninges by tumor cells)and sarcoidosis of the meninges can also cause low CSF glucose values.
