If Sickle Cell Screen is positive, then Hemoglobinopathy Evaluation will be performed at an additional charge (CPT code(s): 83020, 85014, 85018, 85041).
Hemoglobinopathy Evaluation includes: Hemoglobin A, Hemoglobin F, Hemoglobin A2 (Quant), Hemoglobin S, Hemoglobin C, Hemoglobin E, and any hemoglobin variants

Instructions
Maintain specimen at room temperature. Do not centrifuge. Patient age and ethnicity are necessary for proper interpretation. Blood transfusions within the last 4 months may affect results.
Note: This test is not appropriate for patients less than 6 months old due to interference by Hemoglobin F. In these cases it
is suggested that test code 35489 (Hemoglobinopathy Evaluation) be ordered. If tube drawn is shared with lead testing, trace metal-free EDTA (i.e. royal blue-top or tan-top) tube is required.
