Clinical Significance
High Sensitivity Troponin I (HSTNI) assays are used to measure the levels of cardiac Troponin I in the blood, which is a protein released when the heart muscle is damaged.  These tests are approximately 1000 more sensitive than previous Troponin I (TNI) assays.  This allows for the detection of even tiny amounts of Troponin I that may indicate heart conditions such as heart attacks.  The high sensitivity of this assay enables earlier and more accurate detection of heart issues which can significantly impact patient management and outcomes.

Patient Management/Treatment Pathways:
Due to this assay change, there are two new dynamic Patient Management/Treatment Pathways available in Epic to help providers manage patient care with this assay and results. These pathways can be accessed in a patient chart using the Pathways Tab in Epic (see AgileMD Pathways Tip Sheet).
  • Note: Pathways are currently in Draft form and order links will be live 5/1/25.
  • Pathway Names:
Inpatient Chest Pain Evaluation (High Sensitivity Troponin)ED Chest Pain Evaluation (High Sensitivity Troponin)

Static versions of the pathways can also be accessed through:
