Tobramycin Trough

Test Code
TOBRT

Alias/See Also
Nebcin, TOBRA

Preferred Specimen
1.0 mL plasma (0.5 mL minimum)

Instructions
Collect specimen immediately before dose (within thirty [30] minutes).
Verify that the next dose has not yet been given.

Transport Container
PST (light green top). Red top and SST also acceptable

Specimen Stability
Room temperature: 8 hours, Refrigerated: 3 days, Frozen: 1 month

Reject Criteria (Eg, hemolysis? Lipemia? Thaw/Other?)
Grossly hemolyzed specimens are unacceptable.

Limitations
Aminoglycosides structurally similar to tobramycin (e.g. kanamycin) may significantly cross-react with this method. Therefore, the TOBR method cannot reliably be used for patients receiving these antibiotics separately or in combination with tobramycin.

Reference Range
0.5 - 2.0 ug/mL

Clinical Significance
Tobramycin is an aminoglycoside antibiotic. TOBR test results may be used in the diagnosis and treatment of tobramycin overdose and in monitoring levels of tobramycin to ensure appropriate therapy and minimize ototoxicity and/or nephrotoxicity.
