DRAW 1 BLUE NA CITRATE TUBE, MIX WELL, DOUBLE SPIN, SEPARATE PLASMA INTO PLASTIC TRANSFER TUBE,FREEZE RED CELLS IN A SEPARATE TUBE. SEND TO DHMC. WITH COURIER.

PLASTIC TRANSFER TUBE AS WELL AS MANIFEST TO DHMC.

Unlabeled sample, improper temperature during transport. Wrong sample collection.
